- Assist staff with IT and other technology related problems and
work orders.
- Assist in all aspects of planning and implementing network wide
events as well as other events as needed (this includes managing all event purchases and space rental).

Summer Programs – Summer 2013
Build
the Out-of-School Time Network is seeking applicants for literacy coach
positions in out-of-school time programs.
Our Summer Learning Collaborative, in partnership with the United
Way of Massachusetts Bay and Merrimack Valley, will focus on two main goals:
building literacy instruction skills in program staff at our partner programs
and infusing all areas of the program with literacy to improve learning
outcomes and reverse summer learning loss.
Our
project has the unique advantage of bringing together school systems, the
Massachusetts United Way system and after school partner agencies. Working together, we believe that our model
will improve critical relationships with schools and increase the quality of
after school and summer programs by focusing on literacy enhanced curricula and
activities that align with what children are learning during the school
year.
The
literacy coach is a critical part of our team and will be providing on site
support to out-of-school time programs on literacy curricula, infusion, and
instructional skills and strategies as well as supporting the Massachusetts
Curriculum Frameworks. Coaches will work
with a partner program in Boston, Lawrence, Lowell, Lynn, Salem or Somerville,
Ma.
Duties:
Coach out-of-school
time program staff in successful literacy practices for K-3 students, including
the following required activities:
- Assess staff
literacy skills and interests
- Create
literacy infusion plan with staff
- Guide staff in
creating or improving level library as needed
- Review and
select curricula, learning activities, and lesson plans with staff
- Train staff,

This position is

Duties:
Transcribe
a wide variety of clinical reports utilizing dictation, transcription (headset)
equipment. Troubleshoot transcription equipment failure. Place reports into the
electronic medical record. Proofread reports for accuracy, correct grammar and
punctuation. Interact with provider staff to have reports signed. Maintain
confidentiality according to hospital policy, state and federal law. Assist in
other departmental functions as needed.
1. Transcribe material of more than average difficulty, such as medical and
psychiatric reports, and other special reports and file in Meditech according
to established procedures.
2. Proofread typed material for grammatical, punctuation and spelling errors
and for proper format, accuracy and completeness, and submit necessary
completed transcription to Supervisor for coding.
3. Compose routine business letters as may be required.
4. Alert providers when transcription is complete and ready for signature, and
follow-up on delinquent signatures.
5. Per procedure, forward copies to HIS clerk when dictator requests copies to
be sent to DMH site or facility.
6. File records and maintain files of discharged employees.
7. Work in collaboration with Guardianship Coordinator in the preparation of
guardianship paperwork, obtain physicians’ signatures, and communicate as
necessary with DPH Legal Department.
8. Assist in performing various aspects of the discharge analysis and assembly
functions.
9. Maintain confidentiality to ensure patient information is protected in
accordance with Tewksbury Hospital policy, state and federal regulations.
10. Assist in purging medical records from HIS library.
11. Understand and abide by the HIPAA regulations as outlined at Tewksbury
Hospital.

YouthBuild-Lawrence
(YBL) is a comprehensive national youth and community development program for
out-of-school youth operating under the auspices of the Lawrence Family Development
and Education Fund, Inc. (LFDEF). YBL is building on sixteen years of success
in “building homes and rebuilding lives.”
Mission Statement
The YBL Program, a
broad-based collaborative community initiative, assists out-of-school and
unemployed young people between the ages of 16-24 in completing their education
and achieving economic self-sufficiency through empowerment skills, educational
and vocational training, career development, support services, community
service, post-secondary education and/or job placement. YouthBuild-Lawrence
participants develop self-esteem, community engagement and clarify their values
on which to build a successful and industrious life.
Program Components
Education: Participants
receive class instruction with a teacher/student ratio of (8 to 1) and
assistance with pursuing academic goals by working toward a GED or high school
diploma, post-secondary education and career development.
Job Training: Participants
receive onsite carpentry and construction skills training. Presently YBL is in
the process of building two affordable single-family homes that will be
marketed to first-time home owners.
Leadership
Development: Participants participate in various life skills workshops,
guest speaker series and share in the governance of their program which
enhances their development of leadership and interpersonal skills necessary to
become effective community members.
Community
Service: Participants organize, plan and engage in numerous community
service projects. YBL service efforts focus on building and re-habilitating
houses for the homeless and low-income residents of Lawrence.

U.S. Census Bureau, Lawrence Massachusetts is the 23rd poorest city in the
nation. Lawrence, a mill city of 72,000 located about 30 miles north of Boston,
was established as a planned industrial city in the 1840s. Like many
manufacturing cities, Lawrence has been hit hardest over the last 30 years by
the loss of industry, jobs and capital, and remains one of the most
economically distressed cities in the United States. Median household income is
just $27,983, compared to $50,502 statewide. Poverty rates are twice the

Full time members

make additional training opportunities available which include: First Aide, CPR
and OSHA. AmeriCorps members will earn an annual stipend of $24,200.00 equaling
484.00 weekly and are eligible to participate in the AmeriCorps health
insurance program and may apply for childcare benefits. Upon successful
completion of the service year, members will be eligible for a $5,550 education
award to pay off selected existing student loans or to pay for future
post-graduate tuition. Selected student loans may be deferred during the one

uAspire (formerly known as ACCESS) is committed to its
mission of ensuring that every young person has the financial information and
resources necessary to find an affordable path to-and through-a postsecondary
education. Founded in Boston in 1985,
uAspire has grown to become the city’s leading provider of financial aid
advising and its largest scholarship provider.
In 2011, uAspire expanded to the city of Lawrence. The Program
Coordinator Fellow will lead a diverse set of college affordability
services-including direct one-on-one advising and delivery of
workshops-focusing on engaging students and families in after-school hours. The
Fellow will also play a key role in our community-based approach to advising by
strengthening our evening partnerships with several community-based
organizations.
Essential

- Conduct
one-on-one college affordability sessions during after-school hours for all
Lawrence High School complex schools

as the uAspire liaison for all uAspire partners in the Merrimack Valley

Duties:
Transcribe a wide variety of clinical
reports utilizing dictation, transcription (headset) equipment. Troubleshoot
transcription equipment failure. Place reports into the electronic medical
record. Proofread reports for accuracy, correct grammar and punctuation.
Interact with provider staff to have reports signed. Maintain confidentiality
according to hospital policy, state and federal law. Assist in other
departmental functions as needed.
1. Transcribe material of more than average difficulty, such as medical and
psychiatric reports, and other special reports and file in Meditech according
to established procedures.
2. Proofread typed material for grammatical, punctuation and spelling errors
and for proper format, accuracy and completeness, and submit necessary
completed transcription to Supervisor for coding.
3. Compose routine business letters as may be required.
4. Alert providers when transcription is complete and ready for signature, and
follow-up on delinquent signatures.
5. Per procedure, forward copies to HIS clerk when dictator requests copies to
be sent to DMH site or facility.
6. File records and maintain files of discharged employees.
7. Work in collaboration with Guardianship Coordinator in the preparation of
guardianship paperwork, obtain physicians’ signatures, and communicate as
necessary with DPH Legal Department.
8. Assist in performing various aspects of the discharge analysis and assembly
functions.
9. Maintain confidentiality to ensure patient information is protected in
accordance with Tewksbury Hospital policy, state and federal regulations.
10. Assist in purging medical records from HIS library.
11. Understand and abide by the HIPAA regulations as outlined at Tewksbury
Hospital.

3. Provides evidence-based practices in psycho-education and skills training in
areas such as, psychiatric illness management; symptom reduction; dialectical
behavioral support; trauma-informed interventions; motivational interviewing,
relapse prevention, and in addressing co-occurring disorders.
4. Uses recovery-oriented, motivational interviewing, harm-reduction,
trauma-informed, culturally competent, and strength-based approaches to help
clients prepare to engage in a recovery process and to communicate their
strengths, hopes, dreams, and goals to the team and other service providers.
5. Provides comprehensive assessments for all areas of the client’s life as a
member of the client’s Individual Action Plan (IAP) Team; develop a clinical
formulation that will guide the development of the client’s IAP, as assigned by
the Program Director.
6. Contributes expertise to staff members related to clinical, trauma-informed,

7. Assesses all areas of client risk and safety; provide crisis intervention,

10. Ensures recordkeeping/documentation in accordance with Rehab Option and
other internal and external requirements and regulations.
11. Participates in regular quality improvement and utilization management
activities consistent with CBFS policy and practice.

13. Work with clients receiving services to complete assessments and
collaborate with clients to develop their IAP goals. Complete assessments and
service and billing documentation in a timely, thorough, and thoughtful manner.
14. Assess clients’ vocational functioning on an ongoing basis using background
information and prior work experiences utilizing the Individual Placement
Support (IPS) model.
15. With the client’s permission, provide education and support to family
members and gather input about skills, interests, strengths of the client and
ideas for support.
16. In clinical capacity, carries out job duties in a professional manner;
works collaboratively within and across the services system; promotes strong
and positive relationships between DMH and provider staff; and demonstrates a
commitment to professional growth and development.
17. Provides coverage for Supervisor and Program as necessary.
18. Identifies and takes initiatives to problem solve around issues and
concerns that come up in the Program.
19. Perform other related duties as assigned.

Duties:
Provides a range of
clinical social work services to a diverse, high risk, caseload of short-term
and long-term patients of a public, long-term acute care hospital. The patient
population includes individuals admitted for medical stabilization,
reconditioning, active rehab and discharge back to the community. These include
patients with a combination of acute or chronic medical problems with a
neuro-degenerative or substance abuse/addiction problem, as well as patients
with developmental disabilities. Psychosocial issues include Health Care
Proxy/Guardianship, discharge planning, behaviors, sobriety, health insurance,
finances, and legal issues. Responsibilities include crisis intervention,
patient/family education, case management/discharge planning, documentation in
the EMR, interdisciplinary team work, supervision of clinical social work
staff, and referrals to community agencies.
1. Complete a High Risk Screen on all new admissions within 2 working days of
the date of admission to the hospital.
2. Complete a Discharge Planning Assessment on all new admissions within ten
working days of the date that a High Risk Screen identified a patient in need
of post-hospital, continuing care services.
3. Attend rounds as held and conduct other case finding activity to continually
update the assessment of each patient’s overall status noted in the initial
assessment. Initiate appropriate interventions to address and resolve any
psychosocial stressors identified via case finding or referrals from other
sources.
4. Attend regularly scheduled meetings of the interdisciplinary team and be
prepared to input information, advice, observations or suggestions to address
patient care needs/problems that impact their level of bio-psychosocial functioning.
5. Provide or refer patients to individual and or group counseling services as
needed to assist patients and family members to cope effectively with
stressors, crises and conflicts related to illness, disability and institutional
living.
6. Serve as the interdisciplinary team’s liaison contact to the designated
family member or surrogate decision maker to maintain easy and open
communication and promote goodwill and high levels of customer satisfaction
with hospital services.
7. Document social work entries into the electronic medical record (EMR)
according to the hospital and the department’s policies and procedures.
8. Ensure that patients and/or surrogate decision makers receive appropriate
assistance needed to apply for and retain health insurance and disability
income benefits available from state and federal agencies.
9. Initiate discharge planning discussions with patients and/or surrogate
decision makers. Work cooperatively and effectively with interdisciplinary team
members to design and implement a comprehensive plan to meet the patient’s
post-hospital continuing care needs.
10. Confer with patients about post-hospital, discharge plans in a caring,
compassionate and respectful manner. Promote/encourage the active participation
of patients and, or surrogate family members, in discussions about discharge
planning.
11. Educate patients and, as indicated, family members about services,
programs, benefits/resources available from state, federal and private agencies
to meet patients post-hospital, continuing care needs.
12. Develop and maintain an up to date knowledge/awareness of health and social
welfare programs/services/benefits appropriate for adults related to
counseling, rehab, health insurance, disability programs, addiction, group
homes, subsidized housing, nursing homes, assisted living, out patient medical
care.
13. Work with DDS as the primary point of contact for the interdisciplinary
treatment team and coordinate referral information and discharge planning in
cooperation with the designated DDS staff.
14. Coordinate the plans and schedules for durable medical equipment, supplies,
medical appointments, skilled nursing care and rehab services to be in place to
ensure the smooth transition of patient’s discharge from the hospital to the
after-care setting.
15. Provide information, education, counseling services to patients and family
members on matters relating to advanced care directives, health care proxy
rights, legal guardianships, patient rights and medical ethics issues
pertaining to end of life decisions.
16. Ensure that disabled and seriously impaired patients receive appropriate
assistance to resolve miscellaneous/situational needs and problems related to
aging, institutional living and physical/emotional dependency on others.
17. In collaboration with staff physicians, consulting psychiatrists and
attorneys at DPH Legal Office coordinate arrangements to apply for legal
guardianships and Rep payee authority for patients who have recently lost
sufficient capacity to make informed judgments and decisions regarding medical
care and financial matters.
18. Supervise a Social Worker A/B. Meet with supervisee on a regularly schedule
basis to review cases, identify and address problems, assign duties to be
performed according to the employees Form #30 Job Description.
19. Assist the interdisciplinary team to devise, implement and sustain
individual treatment plans to address a range of behavior problems posed by
patients.
20. Comply with all applicable state and federal laws including the Health
Insurance Portability and Accountability Act (HIPAA) regulations which govern
the privacy and confidentiality of information about patients.
Qualifications:
MINIMUM ENTRANCE REQUIREMENTS:
REQUIRED WORK EXPERIENCE:
At least two years of full-time, or equivalent part-time, professional
experience as a clinical social worker after earning a Master's degree in
social work.
SUBSTITUTIONS:
- A Doctorate in social work, psychology, sociology, counseling, counseling
education, or human services may be substituted for the required experience on
the basis of two years of education for one year of experience.
- One year of education equals 30 semester hours. Education toward a degree
will be prorated on the basis of the proportion of the requirements actually
completed.
REQUIRED EDUCATION:
A Master's or higher degree in social work is required.

- Licensure as a Licensed Certified Social Worker by the Massachusetts Board of
Registration in Social Work is required.
